Survey Learning Object is for you to create the survey form, for example, you can create a Survey for your students about the course they are taken. The Survey object works in the same way as FAQ, you need to create the survey first and then insert the survey items one by one through the dedicated form.

1. Repeat the steps 1 to 4 in FAQ LO, but choose Survey in step 4.

Note: Create Survey LO page appears.

2. In the Title textbox, enter the name of the Survey form, eg. Markey Survey.

3. In the Description field, enter the description of the Survey form. Click Insert.

Note: It returns to Course Object page. Notice that the newly created Survey LO is listed.

You are now to create survey items.

4. Click Modify Learning Object button of the Survey Learning Object.

5. You can edit the name and description of the survey form by clicking the Edit button.

6. From the drop down list, choose the type of question for your survey item, then click Add

There are several question types for the Survey LO's.

CH-Single choice

Single choice can have more than two options of answer for the student to choose, but student can only choose one of the options in the question.

1. In the Question's title editor, enter the question.

2. In the Answer textboxes, enter the options' answer of the question.

3. If you wish to add another option for the answer, you can click Add an answer and it will automatically add another answer textbox for you to insert the answer.

4. To remove the answer, click Remove answer button. It will remove the lowest answer from your answer list.

5. Click Insert after you finished key in the question and answers.

MC-Multiple choice

Multiple choice can have more than two options of answer for the student to choose, and student can choose one or more than one option in the question.

The interface of inserting the question and answers is totally same with the inserting Single choice.

BP-Break Page

Break page is for you to separate the question by pages. For example, you can arrange all the Single choice(CH) and Multiple choice(MC) question in a page then insert the page break after all the CH and MC question, then another page are the questions by using the Question extended text option. Below is the example of arranging the survey question.

1. You can arrange the question order by clicking the up and down arrow, or select the row at the Move drop down list and select the order arrangement, then click Move.

2. Click the Edit to edit the question. Click the Remove Question to delete the question.

3. Confirmation of deleting page appear, click Confirm if you want to delete.
